Varicose veins - treatment of perforators using TRLOP

Incompetent perforating veins (IPV) are not commonly treated by most doctors who "do" varicose veins. As a result, incompetent perforating veins are found to be one of the commonest causes of why veins come back again after treatment.

In 2001, Prof Mark Whiteley and Judy Holdstock of The Whiteley Clinic invented TRLOP - TRansLuminal Occlusion of Perforating veins:



The TRLOP technique has been shown to be as effective as the old general anaesthetic SEPS (sub-facial endoscopic perforating vein surgery) procedure - but TRLOP is performed:

• under local anaesthetic
• using a pin-hole only (no stitches)
• as a walk-in walk-out or "ambulatory" procedure

Klippel-Trenaunay syndrome treated with TRLOP and Foam Sclerotherapy

Klippel-Trenaunay syndrome is an abnormality of the veins - often mistaken for bad varicose veins.



This is a case of a patient treated successfully at The Whiteley Clinic using our TRLOP method of closing perforator veins and the foam sclerotherapy.

Previously the patient had undergone open vein surgery and skin laser surgery, both of which had made no difference but had left scars.

Mark Whiteley wins veins debate in New York

This morning, Mark Whiteley was involved in a debate against Steve Elias from the USA. The debate was about whether vein surgeons should treat perforator veins or not.

Mark Whiteley and Judy Holdstock from The Whiteley Clinic invented the method of ablating incompetent perforating veins in 2001 - their technique being called "TRLOP" - or TRansLuminal Occlusion of Perforators.

Steve Elias then subsequently renamed the technique "PAPS" - Perforator Ablation Procedure.

The debate lined up as:

• Perforator Ablation IS Overused  - Steve Elias
• Perforator Ablation IS NOT Overused - Mark Whiteley

The debate from Steve Elias was that he accepted that Incompetent Perforator Veins need to be ablated - nut only selectively.

Mark Whiteley argued that there is very good evidence that Incompetent Perforator Veins can be treated, reduce recurrence of ulcers and excellent circumstantial evidence that NOT treating them is a major cause of recurrent varicose veins.

The audience vote was decisive - 90% agreed with Mark Whiteley that Perforator Ablation is NOT overused.

This is excellent support from international vein experts who agree with one of the most important differences in the treatment for varicose veins and leg ulcers that The Whiteley Clinic and The Whiteley Protocol offers over that offered by most vein doctors and clinics.

Tomorrow afternoon, Mark Whiteley will be giving an invited talk at the Endovenous Forum at 9th International Symposium on Endovascular Therapies in Barcelona as part of the faculty of the meeting http://site2011.atlantacongress.org/ponentes

His talk will be "Ablation techniques for perforator vein incompetence: Methods and results".

In it, he will present the TRLOP (TRansLuminal Occlusion of Perforator) technique that Mark and Judy Holdstock invented in 1999 and the results of the TRLOP technique used with different catheter devices.

He will also explain that PAP (Perforator Ablation Procedure) that "appeared" in the USA in about 2004, following a talk the Mark gave in Miami, is essentially exactly the same procedure.

Therefore to save confusion, it would be sensible if the original name for the procedure was used - TRLOP.

Mark will also be presenting some comparative data between different Radiofrequency devices and Lasers showing which are best to use with TRLOP to ablate incompetent perforating veins.

Celebrity varicose vein diary - Eve Cameron in SHE magazine

The beautiful Eve Cameron (beauty editor and author) has written up her experiences of having her varicose veins treated at The Whiteley Clinic in this month's SHE Magazine - June 2009.

In the article - "Vein Hopes" on page 101 and 102 - Eve explains what made her decide to come to The Whiteley Clinic for her vein surgery.

She then goes on to describe the process that she went through - using EVLA (Endo Venous Laser Ablation), TRLOP (TRans Luminal Occlusion of Perforators - invented at The Whiteley Clinic), phlebectomies (removal of the surface veins) and finally sclerotherapy injections under ultrasound guidance.

Eve has published her own before and after photos as part of the article - showing the improvement that she obtained.

In the article, Eve Cameron does point out all of the things that are usually left out - the bits she didn't like and the cost!!!

However, the article is well worth the read as an unbiased diary of someone who has gone through The Whiteley Clinic protocol and seen our results for themselves.
