Completion of bespoke Endovenous Course at The Whiteley Clinic

Today sees the completion of a very successful bespoke Endovenous Course at The Whiteley Clinic, run by The Clinical Exchange.

Dr Hussein Safar and Dr Abdulaziz Al-Muzaini, leading vascular surgeons in Kuwait, joined The Whiteley Clinic team for the whole range of endovenous procedures including EVLA (laser), RFiTT / RFA (Radiofrequency), TRLOP (perforator vein Closure), ambulatory phlebectomy, foam sclerotherapy and microsclerotherapy.

In addition, we discussed the latest research into how venous problems occur and also the latest treatments for veins and leg ulcers - particluarly complex recurrent varicose veins that had been treated elsewhere using more dated techniques and had recurred.

As with all of the visitors to The Whiteley Clinic courses, we will be keeping in close contact with our new friends in Kuwait providing advice and support whenever needed.

Mark Whiteley speaking at Endovenous Forum in Barcelona

Tomorrow afternoon, Mark Whiteley will be giving an invited talk at the Endovenous Forum at 9th International Symposium on Endovascular Therapies in Barcelona as part of the faculty of the meeting http://site2011.atlantacongress.org/ponentes

His talk will be "Ablation techniques for perforator vein incompetence: Methods and results".

In it, he will present the TRLOP (TRansLuminal Occlusion of Perforator) technique that Mark and Judy Holdstock invented in 1999 and the results of the TRLOP technique used with different catheter devices.

He will also explain that PAP (Perforator Ablation Procedure) that "appeared" in the USA in about 2004, following a talk the Mark gave in Miami, is essentially exactly the same procedure.

Therefore to save confusion, it would be sensible if the original name for the procedure was used - TRLOP.

Mark will also be presenting some comparative data between different Radiofrequency devices and Lasers showing which are best to use with TRLOP to ablate incompetent perforating veins.

Whiteley Clinic research fellow presents at National meeting

George Badham, one of the Whiteley Clinic Summer Research Fellows from 2010, presented his research today at a National meeting in London.

His research was looking at how to optimise our treatment of Varicose Veins using the new RFiTT device from Celon and Olympus.

This device, that has been used for varicose vein surgery in Europe for some years, has only had success rates of about 90% - which is not good enough for The Whiteley Clinic. It has also associated with difficulty in treating due to the device sticking.

George Badham's research was to look into Mark Whiteley's Theory that the device was fine, but the technique that was being used was wrong.

Using a bench top model, George Badham showed not only that the current technique is flawed but if The Whiteley Clinic method of using RFiTT is used, all of the problems are overcome.

This work has been submitted for publication now - the reference for today's presentation:

World's first ELSA - Endoscopic Laser Sweat Ablation

This morning Mark Whiteley performed the world's first ELSA - Endoscopic Laser Sweat Ablation.

Having performed the first Laser Sweat Ablation (LSA) in the UK as a permanent cure for under arm sweating in March 2009, Mark and his team at The Whiteley Clinic have been modifying and improving the LSA technique over the last 2 years.

Today saw the unveiling of the latest development - ELSA. Using a keyhole surgery or "endoscopic" camera, Mark and his team were able to monitor the Laser Sweat Ablation procedure, making sure that the treatment was effective as possible.

ELSA should continue to improve the results of this permanent, walk-in walk-out procedure to stop underarm sweating.

Sunday Times Article - Mark Whiteley quoted

Mark Whiteley was quoted today in the Sunday Times (The Sunday Times, 6th March 2011, Page 7 - "Chill out, chubby: ultrasound and freezing to end yo-yo diets").

Mark and his colleague, Vicki Smith director of Absolute Aesthetics, have been the UK centre for a multi-centre international trial of Liposonix over the last year.

Liposonix is a HIFU (High Intensity Focused Ultrasound) machine that has been designed to destroy fat deposits on the abdomen non-invasively. It is not used for removing large mounts of fat - only dieting or liposuction will do that. However for stubborn areas of fat on the belly, or for love handles, the HIFU can be used to destroy these fat cells and therefore allow body countouring.

Mark Whiteley has been investigating the use of Liposonix for over a year and has presented his work at IMCAS in Paris in 2010 and recently at The Royal Society of Medicine (see earlier in this blog).

Venous leg ulcer cured by The Whiteley Protocol

Saturday, 15 January 2011

The Times - Magazine Article

Mark Whiteley was quoted in The Times Magazine today - 15th Jan 2010.

In an article by Sarah Vine called "Purple vein, purple veins" (page 51, The Times Magazine) Mark Whiteley explained the difference between "Thread Veins" of the face - and those of the legs which need far more in-depth understanding to treat successfully.

Thread veins on the face can be very successfully treated by several methods, available at Absolute Aesthetics (www.AbsoluteAesthetics.co.uk).

Leg thread veins however need a specialist Duplex Ultrasound Scan to find out what the underlying cause is and to make sure the correct treatment is planned - which is often to hidden varicose veins deep inside the leg and unseen under the leg thread veins. The Whiteley Clinic in Harley Street and Guildford specialises in these scans and treaments (www.thewhiteleyclinic.co.uk)

Text book varicose veins and venous reflux - by Mark Whiteley

Mark Whiteley has just finished a new text book which explains how leg veins work - and is looking for a publisher to publish it. It includes:

- the new understanding as to why varicose veins, venous eczema and leg ulcers occur
- the underlying cause for leg thread veins as well as varicose veins other venous conditions
- the rationale for the new treatments

This text book includes new concepts that have only been identified in the last few years - and it explains why so many of the "old" ideas of varicose veins are wrong - such as:

- Why varicose veins are NOT just cosmetic
- Why pregnancy DOES NOT cause varicose veins
- Why some tests doctor use do NOT show venous problems - even when they are there
- Why VENOUS HYPERTENSION doesn't actually exist

It explains for the first time:

- "Phase 1 and phase 2 passive reflux"
- Active reflux
- The "double whammy effect"

There are over 50 figures to make understanding easy - so that everyone who is interested in veins - doctors, nurses, medical students, therapists and beauticians - can all learn things that will help them in their everyday approach to venous problems - whether it be assessing varicose veins, treating thread veins or diagnosing leg ulcers and skin discolouration.

This book will be the first in a series of books covering the whole of phlebology. When a publisher is found and the book is widely available, it will be announced here.

Testimonial for Mark Whiteley's veins courses

Mark Whiteley runs regular vein courses at The Cllinical Exchange - from thread veins to EVLA, Radiofrequency Closure, Foam Sclerotherapy, perforator vein surgery (TRLOP), ambulatory phlebectomies etc.

All courses are supported by full discussion of the latest relevant research and development in the world of veins.

Last week, one of our delegates flew in from Croatia for a course - and has just sent this by e-mail:

"Coming to your course was a wonderful experience for me. What I really needed were all those tricks and tips that you generously provided to us. I was feeling very comfortable at your clinic, and all the staff was eager to show us what we were interested in.

Dr Whiteley, you've opened my horizons and now I am reorganizing my future plans. Anyone who will ever work with me in the field of veins, will be sent to you for proper education. I am looking forward your phlebology institute because many of us will benefit from it.

Take care of your health ( coffee!!!!), to be able to help many of us, patients and doctors.

Good luck to you and your team."

Another leg ulcer cured with The Whiteley Protocol(TM)

This lady, in her 80's, attended The Whiteley Clinic in September this year, with a left leg ulcer (see first photo). She had been "treated" by her local doctors and nurses for over a year with bandages, dressings, elevation and she had been given a "Doppler" test. She had been told there was nothing else that could be done for her.

As with a great many people that we see at The Whiteley Clinic with leg ulcers, the ulcer appeared to do well when it was dressed and bandaged, but broke out again as soon as the bandaging was stopped.

No one had offered her a cure.

Having heard about The Whiteley Clinic from one of her carers who had seen us sure another lady with a leg ulcer, this patient came for a scan and consultation.

Using our specialist team, we identified the underlying problem and treated her in one local anaesthetic "walk-in walk-out" procedure - as in The Whiteley Protocol (TM).

8 weeks after the local anaesthetic keyhole procedure, the lady is walking without any discomfort from her ulcer, and her leg ulcer is almost completely healed. There is one small area that has a superficial scab that will heal fully in the next 2 - 4 weeks.

She does not need any dressings, bandages nor stockings - and this cure should be permanent as the underlying cause has been corrected.

New website on Venous Eczema

We are delighted to launch the new Venous Eczema website:

Venous Eczema is one of a host of under diagnosed conditions related to varicose veins and venous reflux (hidden varicose veins).

Many people have red or brown stains on their legs due to venous eczema. Unfortunately as the underlying venous reflux is often hidden away (hidden varicose veins) GP's and nurses often just give steroid creams or other creams to try to make it feel better.

Although this stops any itching for a while, it doesn't take away the underlying cause - that of the venous reflux (or hidden varicose veins) - meaning it continues to worsen and head towards leg ulceration.

This website explains the problem - gives example pictures - and shows the right treatment that can cure the problem:

venous duplex ultrasound and the correct surgery as recommended by The Whiteley ProtocolTM.

RFiTT in Grazia magazine

This week Grazia magazine is running an article on the new RFiTT technique for treating varicose veins - and it quotes Mark Whiteley whose clinic has been doing research to improve the results of this technique.

RFiTT is a radiofrequency device that is used through a keyhole (or pinhole) approach, that closes Varicose Veins using heat. The treatment is all controlled by ultrasound to make sure that the target vein is completed destroyed.

Research on RFiTT performed this summer at The Whiteley Clinic by George Badham - Whiteley Clinic research fellow - won 3rd Prize at the IVEC and ISVS in Milan, in October this year.

Hand veins - Endovenous laser closure of hand veins

Prominent hand veins have always posed a problem for venous surgeons. Many patients ask for treatment for prominent hand veins - but until recently, very little has shown to be effective or cosmetic.

A recent advance in removing these hand veins has been "endovenous laser ablation of hand veins".

This afternoon, two cases were performed by Mark Whiteley at Absolute Aesthetics (www.AbsoluteAesthetics.co.uk) the aesthetics division of The Whiteley Clinic.

Using only local anaesthetic, the whole procedure took about 30-45 mins. The patients came in an hour before the surgery for marking and anaesthetic cream to the skin.

The procedure itself was performed in a walk-in walk-out local anaesthetic theatre - with pinholes only where the laser fibre was inserted into the veins.

Both patients were discharged home without any complication wearing compression gloves which they were instructed to wear for one week.

The advent of endovenous ablation of unwanted hand veins is a major step forward in hand rejuvenation. When used in combination with our other techniques such as hyaluronic acid filler for bony hands and IPL or skin preparation creams for sun damage, hand rejuvenation should now become much closer to the ideal that patients want.

Endovenous laser ablation of hand veins under local anaesthetic is now available at Absolute Aesthetics as part of a hand rejuvenation package.

Steam vein sclerosis - SVS

A new technique is being reported for the treatment of Varicose Veins (Daily Mail, 19th October 2010) called Steam Vein Sclerosis (SVS).

This technique follows the same principles of Endovenous Laser Ablation (EVLA) and Radiofrequency Ablation (RFA) of varicose veins - but claims to be quicker.

It is well understood that heating veins destroys them permanently - and with Laser and Radiofrequency Ablations this heat energy can be measure precisely - a measurement called the LEED (Linear Endovenous Energy Density).

The potential problem with the steam is that it would appear difficult, if not impossible, to measure this energy given to the vein wall.

This might the reason that the report says that only 13 of the 20 veins remained closed after 1 year - very poor results compared to The Whiteley Protocol (which uses EVLA or RFA depending on vein anatomy) which has shown closure rates of over 99% at 5 years.

It will be interesting to see if research can improve on these results.

International prize for Whiteley Clinic research student in Milan

George Badham, the Whiteley Clinic Venous Research Fellow summer 2010, won the prize for third place at the International Vascular and Endovascular Course (IVEC) and the European Congress of the International Society of Vascular Surgery (ISVS) in Milan with his presentation about treating varicose veins using the keyhole surgery technique RFiTT.

George Badham, the Whiteley Clinic venous research fellow summer 2010, being congratulated on his 3rd place prize at the IVEC in Milan by Professor Giorgio Biasi

George, who is a third-year medical student at Cardiff University, was the only medical student presenting, with all of the other presenters in the prize sessions being qualified doctors specialising in vascular surgery. Despite being over a decade younger than the other presenters shortlisted for the prize, the quality of Georges research and his excellent presentation to an international audience of vascular experts, secured him enough votes to win third place.

George Badham, the Whiteley Clinic venous research fellow summer 2010, giving his presentation which won him 3rd place prize at the IVEC in Milan October 2010

The Whiteley Clinic has a strong research tradition and offers research projects to students who wish to get to medical school, medical students and also junior doctors. Each year, one or two applicants are accepted and are given one or more projects to complete, depending on the time they have available and also their own motivation.

Despite careful supervision, not all research fellows manage to get their research completed. It takes considerable dedication to see a research project through to completion and then on to presentation and publication.

George Badham, the Whiteley Clinic venous research fellow summer 2010, being awarded his 3rd place prize at the IVEC in Milan by Professor Giorgio Biasi

This summer, two research fellows were appointed, both of whom are exceptionally hard workers and have completed their projects.

George Badham's research was a study using a benchtop model to simulate keyhole treatment of varicose veins using the RFiTT technique. He showed that the technique that had been originally recommended by surgeons elsewhere in this country and Europe had inadequate affect, making the technique appear less successful than it could be.

Mark Whiteley giving his invited lecture on Foam Sclerotherapy for the Treament of Varicose Veins at the IVEC in Milan, October 2010

With his supervisor, Mark Whiteley, they developed and prove that changing the power settings and the technique of applying the power to the vein resulted in optimal conditions likely to destroy any vein treated this way.

This project will be submitted for presentation in the UK to inform UK surgeons how best to use this technique, and hopefully this presentation will be in the first half of 2011, depending on which meeting it is presented at.

The results will be published in a peer-reviewed journal in the near future.

Mark Whiteley was also an invited lecturer at the same meeting, speaking on the current thoughts on Foam Sclerotherapy in the treatment of varicose veins.

Embarrassing Bodies - Channel 4 - Laser Sweat Ablation

This evening Mark Whiteley was featured performing an operation for arm-pit sweating (axillary hyperhidrosis) on Embarrassing Bodies - Channel 4.

Faye, the patient, was a 19 year old girl who had previously been on Embarrassing Bodies with axillary hyperhidrosis. She had been sent for Botox - which had only worked for 6 months each time.

She wanted a permanent cure - so she was referred to Mark Whiteley at The Whiteley Clinic for Laser Sweat Ablation. She had this walk-in walk-out procedure under local anaesthetic and was on her way home 2 hours later.

On Embarrassing Bodies she said how pleased she was with the result. She had 2 tiny scars on each side, did not have to worry at all about sweating anymore and had gone out and bought a whole new wardrobe of clothes!!!

Laser Sweat Ablation was introduced into the UK by Mark Whiteley who performed the first Laser Sweat Ablation procedure on 5th March 2009. He and his researcher, Sophie Strong, are continually modifying and improving the LSA technique to improve the results.

Tetren - Game by Andrew and Mark Whiteley and friends from 1992

Andrew Whiteley, Mark Whiteley's brother, wrote a computer game in 1992 called Tetren. Andrew did all of the programming, Mark Whiteley drew and animated all of the graphics and Mike Kingham composed the music.

Many hundreds of people played the game via shareware and contacted us to say how much they loved it.

As it was written for the Amiga 500 computer, it has not been available to run on modern computers for many years. However, with a new emulation program, Tetren can now be played on normal modern computers!!!

If you would like to play - click on the two links below and save them to your desk top. One is a zip file which contains all of the files that you will need - the other is a set of instructions on how to get Tetren working.

We all hope that you have as much fun as we had so many years ago.
