Recurrent varicose veins - pelvic venous reflux is ignored by most

Alice Whiteley is one of two Whiteley Clinic Summer Research Fellows who have had their research selected for presentation at The Medical Acorn Foundation (National Medical Student Research Conference) in Leicester on 13th October 2012.

Presenting her research into the cause of recurrent varicose veins operated on elsewhere 
Alice spent her summer analysing the results from patients seeking help from The Whiteley Clinic having had varicose vein surgery elsewhere and then getting recurrent varicose veins.

She identified the causes of their recurrent varicose veins, and found that pelvic venous reflux is one of the major causes, a problem that has previously been ignored.



 
Alice's research presents powerful evidence for why the pelvic veins need to be scanned and, if refluxing, treated, to reduce the risks of recurrent varicose veins in the future.

Her work has been submitted for an American meeting next year and will be submittied for UK vein meetings next year, before submitting for publication.

10 year results of Radiofrequency Ablation of Varicose Veins


Daniel Taylor is one of two of The Whiteley Clinic Summer Research Fellows from 2012 have had their research into varicose veins accepted for presentation at The Medical Acorn Foundation (National Medical Student Research Conference) meeting on 13th October 2012.

Daniel Taylor has spent this summer recalling pateints who were treated over 10 years ago at The Whiteley Clinic with the VNUS Closure Radiofrequency Ablation system and then analysing all of the 10 years scans of their legs.
 
He has been finding out what the results of this technique are and, if there were any problems, where they came from. The results appear to be the first 10 year results available from endovenous surgery.
 
Daniel has also submitted his research for an American Venous meeting, and will be submitting the same for a UK venous meeting prior to submitting the results for publication.

Laser closure of large varicose veins - with radial EVLA


Endovenous surgery (or "keyhole surgery") is taking over from open surgery for varicose veins, as this approach has a great many advantages. ONe of the biggest is the fact that endovenous surgery is performed under local anaesthetic and so the patients are walking home an hour after the procedure.
 
Although "normal" sized varicose veins are easy to treat by experienced vein experts with endovenous techniques, there has always been a concern about the maximum size that can be treated this way.

These images are ultrasound pictures taken yesterday of one of our patients treated 5 weeks ago at The Whiteley Clinic. The man had a vein that was 31mm in diameter (normal size in varicose veins at this point is between about 4-12mm).

The veins was successfully closed by using a side-firing laser and passing the laser backwards and forwards thorough the dilated segment four times.

By using novel techniques, we have not yet had a patient with varicose veins that we cannot treat with endovenous techniques under local anaesthetic.

2012 Pre-medicine taster course

We are delighted to announce that the 2012 pre-medicine taster course will be starting at The Clinical Exchange in Guildford soon.

Young people who are thinking of medicine as a career are taken through 8 weeks of medical lectures and practicals run by Consultants and GP's.

Delegates get to use ultrasound, practice stitching, get taught how to take blood and other practical skills to see if they like Medicine and can see it as their future career.

The course ends with a mock medical school interview to help delegates prepare for applying to a medical college.

The course runs on Wednesday evenings during the Autumn term. There are a few places left for any interested students.

Removal of arm veins - a "Life changing" operation


A lady contacted The Whiteley Clinic earlier this year by e-mail, showing enlarged arm veins on both sides. She was desperate for removal of the veins as the veins stood out so much.

The photos that she e-mailed are below:

After much discussion, we agreed to perform Ultrasound guided removal of the veins, under local anaesthetic.

2 months later, the patient is delighted with the results:

Whiteley Clinic Prestige Stakes 2012 at Goodwood

This year's Whiteley Clinic Prestige Stakes, held at Goodwood on Saturday 25th of August, was a fabulous affair - despite the cloudy weather. Immaculately organised by Graham Smith of RT-Design, the day was a great success and was enjoyed by all:

The day started off with a flight into Goodwood with Capt. Paul Andrews of the Phoenix Helicopter Academy, based at Goodwood but covering the whole of southern England.

Research at The Whiteley Clinic

The Whiteley Clinic is very well known for the excellent results it gets for the treatment of varicose veins, venous leg ulcers and other vein related conditions.

One of the major reasons is that, unlike other "vein clinics", The Whiteley Clinic has an ongoing research program. By performing our own original research, we are able to stay at the forefront of the investigation and treatment of venous disease.

Our findings are then incorporated into The Whiteley Protocol(TM) making sure that all of our patients get the best possible results, regardless of which of our doctors sees and treats them.

In Autumn this year we are going to sponsor a second PhD student, who will be involved in venous research between The Whiteley Clinic and The University of Surrey.

The Whiteley Clinic Prestige Stakes, to be held at Goodwood this coming Saturday 25th August, is attracting the attention of P J Prendergast who may run Nandiga.

Healing "incurable" leg ulcers with laser surgery

Leg ulcers, due to venous disease, can be cured with endovenous laser ablation (EVLA) surgery under local anaesthetic in the majority of cases - despite most patients being told that they are "incurable".

In a research paper about to be published in the peer reviewed journal "Phlebology", 85% of venous leg ulcers were completely cured, with 52% not even needing stockings or compression afterwards. 98% of all leg ulcers were improved clinically.

All of the patients had been told that there was "no cure" available for them and they were told the only treatment option was compression bandaging and dressings.

All of the patients decided to come to The Whiteley Clinic for a second opinion - and using The Whiteley Protocol (TM) the majority were cured by laser and foam surgery performed under local anaesthetic.

The 2012 Whiteley Clinic Summer Research Fellows are now in post and coming to the end of their research.

Alice Whiteley, Charlotte Thomas and Daniel Taylor (all pictured above) are performing research studies and audits into varicose vein treatments.

Their research includes projects on:
  • The causes of recurrent varicose veins from other vein clinics and practices
  • The evolution of endovenous surgery and minimally invasive vein techniques
  • The long term results of thermoablation of the Great Saphenous vein
All of these projects will be presented and submitted for publication to try to help the world of Phlebology (or Vein Treatments) to move forwards a bit further.

Leg ulcer recurring over 19 years curable by Whiteley Protocol

Leg ulcer recurred over 19 years due to inadequate assessment.
Found to be completely curable under local anaesthetic by The Whiteley Protocol
This 58 year old lady came to The Whiteley Clinic. She had developed a right leg ulcer in 1993 and ever since then, the leg ulcer has kept recurring despite all treatment.

Her local surgeons had "operated" on her varicose veins on 2 different occasions and had told her that nothing more could be done for her surgically.

She has spent the last 19 years in tight compression bandages which slowly allow the ulcer to "heal" - only to recur shortly after the bandages are removed.

Desperate to be cured, she even went privately to a local "expert" surgeon who told her that the best thing to do was to give up work and stay at home with her leg up!

Her son heard about the high success rate of curing venous leg ulcers at The Whiteley Clinic and suggested she come to our Whiteley Clinic in London for assessment.

Her tests, performed by the Specialists at The Whiteley Clinic, showed that the previous 2 operations had completely missed the relevant vein - so in efffect she had never had any surgical treatment at all!

She is easily curable under local anaesthetic using The Whiteley Protocol -  in her case a combination of radial firing EVLA (endovenous laser), TRLOP (closure of perforators) phlebectomies and Foam scerotherapy.

Had she been referred to, and fully investigated by, a vein specialist earlier - rather than her doctors and nurses just assuming that the previous surgery had been successful in dealing with the underlying veins - then she wouldn't have had to suffer so much pain and inconvenience over the last 19 years.

Curable venous ulcer delayed by inappropriate referral

This patient came to The Whiteley Clinic with a leg ulcer on the right leg, with brown skin staining around the lower leg and visible varicose veins on standing.

Despite having a classic venous leg ulcer, he was referred to a plastic surgeon who charged him to say that the ulcer wasn't malignant - and despite several hundreds of pounds, no treatment was suggested or offered!!

When he came to The Whiteley Clinic, a consultation and examination revealed a previous vein operation about 10 years ago using the old "Stripping" method which we now know allows veins to grow back again - causing the same problem to recur (www.veinsstripping.co.uk).

Duplex Ultrasound report - The Great Saphenous vein has been stripped before - but has regrown in the mid portion.
The Small Saphenous Vein is huge (14mm) and there is reflux in Anterior Accessory Saphenous Vein and multiple incompetent perforators.
Patient can be cured by EVLA and TRLOP under local anaesthetic. 
The combination of brown discolouration of the skin at the ankle, a little swelling o the ankle and a leg ulcer made it virtually creating the problem was a venous leg ulcer - and therefore curable in most cases using The Whiteley Protocol(TM).

A venous duplex ultrasound showed that the Great Saphenous Vein had partially grown back again causing problems, and further venous reflux in the Small Saphenous Vein, Anterior Accessory Saphenous Veins and 4 incompetent Perforating Veins were causing the leg ulcer (see "Understanding Venous Reflux - The Cause of Varicose Veins and Venous Leg Ulcers").

All of these veins can be cured under local anaesthetic with a combination of Radial Firing EVLA for the long veins and TRLOP of the incompetent perforating veins.

Mark Whiteley demonstrates how to perform ultrasound guided Seldinger access of the Great Saphenous Vein for "Keyhole Vein Surgery" - Medically known as "Endovenous Surgery". This technique is performed under local anaesthetic.

It is an essential part of endovenous surgery - whether it is for Endovenous Laser Ablation (EVLA), Radiofrequency Ablation (RFA) or for one of the newer techniques such as Venaseal Glue closure of the veins.

Venaseal "superglue" for Varicose Veins at The Whiteley Clinic

This afternoon, Mark Whiteley of The Whiteley Clinic performed 2 procedures using the Venaseal "Superglue" for Varicose Veins made by Sapheon.

Both cases passed without hitch and both patients walked home within an hour of the procedure.

A scan performed straight after the minor operation showed the vein was closed right up to the top junction in the groin - precisely where the problem arises from.

Untreated pelvic veins may lead to Caesarian Section

This very interesting patient came to seek advice at The Whiteley Clinic. She was 4 months pregnant on her third pregnancy.

She had large varicose veins arising from the inner aspect of the leg next to the vulva (para-vulval veins) and from the buttock, leading down into large varicose veins of the leg.

The large varicose veins continued upwards into the vulva and vagina, making her obstetrician worried about bleeding during a normal delivery and leading him to suggest that a caesarian section might be needed.

Unfortunately this lady had seen these veins during her second pregnancy, but they weren't recognised as a sign of pelvic venous incompetence (or pelvic congestion syndrome) and so she was not referred for investigation and treatment.

As such, she is now in this dilemma with her third pregnancy. Had she been referred for treatment when the veins were first noticed, we could have fixed the underlying problem and she would not be facing the possibility of a caesarian for the risk of bleeding.

At The Whiteley Clinic we continually try to raise awareness and interest into this and other venous conditions, so that patients know that these conditions can be serious and also that treatment is available in specialised centres.
